Cover Page for Normal Document Recording with First Page Information Missing
If your document does not meet the requirements below, you will need to add a cover page filled out completely with all the pertinent information pertaining to the document being recorded. The cover page will act as the first page of your document.
The following information is required to be on the first page of the document:
1.Name of the transaction
2.Names of parties (grantor - grantee)
3.The name and address to whom the document will be returned (for mailing purposes only)
4.The true and actual consideration (for instruments conveying or contraction to convey fee title to any real estate and all memoranda of such instruments)
5.The tax statement name and address where the taxes are to be sent (for instruments conveying or contraction of convey fee title to any real estate and all memoranda of such instruments)
6.Satisfaction of Order or Warrant
