Forms Needed for the Travel Consultation and Travel Immunization Appointment
"Traveler Vaccine Screening Form" and "Vaccination Requirements Listed by Country". These materials were designed to assist you in determining what vaccinations you need for traveling to foreign countries.
What to do Before Your Travel Consultation and Travel Vaccination Appointment at Jackson County Public Health
1. Please locate the country or countries you are traveling to on the attached file. Then note the vaccination required or recommended for that country.
2. Next, complete the foreign travel immunization screening form. In the middle of the form, note the vaccinations that you have decided to receive.
3. Bring the completed form to the Health Department at least 5-6 weeks before your trip is scheduled, if possible.
Note: Some vaccinations require a series of two dosed divided by several weeks to one month of time. Other vaccinations are only one dose and are good for a number of years or for a lifetime. Please refer to the information on the assessment form for the specific vaccines you are planning to receive.
Travel vaccine office visit fees apply: $31.00 per traveler; $46.00 for two or more in the same travel group when consultation is provided as a single group. Many insurance companies do not cover the cost of travel immunizations. Payment is expected at the time of the consultation for office visit fee and cost of the immunization that is not covered by health insurance.
