
School Immunizations
Immunizations are required by state law for children and students in attendance at public and private schools, preschools, childcare facilities and Head Start programs in Oregon.
Nearly every facility that provides care for a child outside the home requires immunizations or a medical or nonmedical vaccine exemption to stay enrolled.
If you do not meet state immunization requirements, you will receive a letter stating that your children must be immunized or will not be admitted to school starting on the school exclusion date.
Vaccines keep your child healthy and protected from serious diseases.
Parents
- Review the immunization requirements (below) and have your child immunized. * Fill out a Certificate of Immunization Status Form with the immunization dates for your child. You may also request this form from your child’s school or childcare facility.
- Sign the form and turn it in to your child’s school.
If you receive an exclusion notice, contact your medical provider or Jackson County Public Health’s Immunization Clinic to have your child immunized. Children who are not up-to-date on their immunizations by February 15, 2023 will be excluded from school until their immunizations are up-to-date.
School and Childcare Facilities
- There must be one Certificate of Immunization Status Form or ALERT record on file for each child.
- Report children with incomplete or non-existing immunization records to Jackson County Public Health Department by January 11, 2023.

Important Dates
First day of school or at a certified child care facility - All children must show proof of one dose of all required immunizations. Exception: Students who transfer from another school in the United States have 30 days to have their immunization records transferred to the new school.
January 11, 2023 - Deadline for schools and child care facilities to send us their first report.
February 1, 2023 - Health Department mails exclusion orders to children's families who are not up-to-date on their immunizations.
February 15, 2023 - Exclusion day. All children who are not up-to-date or complete on their immunizations will be excluded from their school or child care facility.
February 27, 2023 - Deadline for schools and child care facilities to send us their final report.
